Using 12.0.3 Server and client (Noticed on Mac but have replicated on Windows) and when I try to upload the new clients into FileWave I only get the first row and only the Client Name and Comment. I never seem to be able to get the MAC address to import for these windows clients. I can manually add it after, but that defeat the purpose. I have attached a screen shot of what I get. My SysAdmin tried to do the same thing on his Windows 10 computer with the same version client and got the same error. Is this a known problem?

Seems like there's something funky with the formatting of your file maybe because it works fine for me. I'm guessing it's a carriage return line feed issue after the first line, as that's the only one that shows up in the import. Other possibility is there's a syntax error somewhere, e.g. a comment is not commented out correctly. I'm on a 12.0.3 server also, downloaded the template, added 2 entries (1 Mac & 1 PC), and reimported. All the devices in the file showed up with comments; my new entries are denoted by the red box in the screenshot.

Could you attach your import file and remove all device entries after the 1st 5 or something like that so we can take a look.

Information was scanned into a Google Doc and then downloaded as a Excel file. Saved out of Excel to Tab-deliminated text file.

There's definitely something screwy with the line endings. That's causing your problem with only the 1st line showing up in the import. Don't know WTH is up with the missing middle column for the comment. Worked around it but don't know why that happens. Anyways here's the convoluted steps I went through.

1. Download a copy of TextWrangler from the Mac App Store.
2. Open your import file with TextWrangler and save it with Windows CRLF line breaks.
3. Copy the the contents of your import file to the clipboard.
4. Download the import template from the FileWave admin console.
5. Open the import template with TextWrangler, scroll to the bottom of the document, delete the sample computer entries, paste in the contents of the clipboard, and save the document.
6. Try importing the list of devices again via the import template and you should get this.

You were almost right from my end. There is a problem with the default encoding that had be corrected. As a result it was not just a reassertion of CRLF that was necessary, but also of Unicode-8 as well. I have reconfigured TextWrangler to force these settings going forward since this is about the only thing I currently use it for that is wacky in that way. Thanks a lot for the assist. I have to ask though if you think this is something from Google's end, FileWave's end or just a lost in translation kind of thing? My money is on Google. Curious to hear your thoughts.

Pretty sure it's on Google's end. If you run "file <name_of_file> in macOS here's what you get.
MacBook-Pro:Desktop baotran$ file ThinkCentresv2.txt
ThinkCentresv2.txt: ASCII text, with CRLF line terminators
MacBook-Pro:Desktop baotran$ file desktop_clients_template.txt
desktop_clients_template.txt: ASCII text
