I am trying to install a Device Based license. The client info shows it associated, but the app does not show up on the client. The client is a MAC with OSX 10.12. The logs do not show any errors relating to installing the App.

Have you bound the client using MDM?
I'm pretty sure it needs to be bound using MDM to have the device based license enabled properly.

Not sure what you mean "bound the client to MDM". It is enrolled and managed. The only bind function is for Active Directory.

The client needs to have the MDM profile installed. You could do this by having your computers enrolled through DEP or either manually installing the profile from the Enrollment URL - Step 2 Or following the steps in this link to deploy the MDM profile via Fileset:

The directions say to download the profile from "Enroll iOS Device". This is for a desktop is that correct?

Yes trandolphRGSD, the directions are correct, the MDM profile is the same for iOS devices and macOS.

This worked. I just opened the IOS enrollment url and manually enrolled, but the app now installed. Thanks for the help.

I'm glad it worked out for you. Of course I meant enrolled (and not bound) to the MDM.
